Plan of Care Visits

Evidence Base:

"Collaborative medical rounds, implemented last spring, help reduce patient length of stay, readmissions and adverse events, and improve teamwork (...) For caregivers, plan of care visits improve teamwork, interprofessional communication and job satisfaction, and reduce inefficiency (including calls to physicians) and burnout."

Relevance to the Canadian Context:

It is relevant in Canada however teamwork has to be prioritized and with staffing shortages I am unsure how feasible this would be.
